Tammy Sirmons is a native Floridian, wife, mother, grandmother, and sibling. She is a registered nurse with over 20 years of experience and the owner and operator of an in-home chronic illness management and support company, Gracefully at Home Caregiving.

As the Area Director of Southeast Florida Elder Care Planning Council, she aims to educate the community about all things related to long-term care planning.

Her journey in the healthcare field has always been deeply rooted in her passion for caring for others, particularly older adults. With years of experience, she has witnessed the profound impact that compassionate care can have on individuals' lives, serving especially those who have paved the way for us, as well as for their families.

Her commitment to enhancing the quality of life of those she serves is the driving force behind her team’s mission to provide personalized, compassionate support in the comfort of their homes, allowing them to “age gracefully.”

Each day she strives to honor their stories and dignity while ensuring they receive the highest standard of care. She has lived the journey of family caregiving as she cared for both of her parents during their later years of life. As she puts it, “my work isn’t just a profession, it’s a heartfelt calling to give back to those who have given so much.”
